What is the Process?

• A rental agreement will be signed specifying terms and conditions of the private mailbox rental.
• A USPS Form 1583 must be completed. This form will be provided and requires two forms of ID for a current photo and current address.

• Acceptable Photo ID includes:
• US State/Territory/Tribal Drivers or Non drivers ID Card (Drivers License or State ID)
• Uniformed Service ID
• US Access Card
• US University ID 
• Passport
• Matricula Consular
• NEXUS Card
• U.S. Certificate of citizenship or naturalization
• U.S. Permanent resident or other ID card issued by the U.S. Citizenship and Immigration Services.

Acceptable Current Address ID includes:

• US State/Territory/Tribal Drivers or Non drivers ID Card
• Current Lease
• Mortgage or Deed of Trust
• Vehicle Registration Card
• Voter Card
• Home or Vehicle Insurance Policy
